3D_HWRAssociationError

I have been creating new hardware and I am getting the hardware to showup but it is always coming in on the 3D_HWRAssociationError layer. There is no machining and irt is where I want it? Obviously there is somehthing I am doing wrong but what are the possible reasons that this could be happening for further investigation?
